It can be both, depending on your system. If you design it right, it's indeed really quiet.
As soon as you make use of a radiator (something I'd advice), you need to make use of fans, to cool the radiator. By running them at 7V, you can run them efficient, by pretty quiet.
Some watercooling setups make lots of noise. This happens when using heavy pumps and noisy fans. In most cases, these systems perform extremely well (besides the sound production ofcourse) or are badly designed.
Most watercooling kits are built to be quit btw..
