Most of times the cause is a low quality disc. Indeed, one of the most frequent issues of low quality media is that after the burn the disc is unreadable.
Another possible cause is that the drive is damaged. Slim drives are more prone to become damaged for various reasons.
To exclude a hardware damage, you can do a test with a proven quality disc like Verbatim.
To exclude a bug in the burning software, you can do a test with a different software. I suggest ImgBurn
