The problem is not really down to anyone person, people or company, its more all of our fault, and a by-product of consumerism.
The problem is that people want to pay the least amount for something, it matters not what it is, most just want to pay less for it, and because of this companies go to manufactures and ask for something to be made in the cheapest way they can. Just as Wizzu said Phillips just go to a media maker and ask for however many millions of DVD's they can get for xxx amount of money, company A, says yes we can do this many or company B, says we can do that, or you can have these (i.e. cheaper media) and more for this amount.
I am sorry, but this is just a fact of life, "you get what you pay for".
I feel that I was quite lucky because I have always used Verbatim, from eight and five and a quarter inch floppy disk down to the smaller ones, and have always tried to stick with them. So when I moved on to CD's then DVD's they were the media I got and the few times I gave another maker/brand a go they never seemed to be as good, and in the long term cheaper media costs more in the long run because you will have problems and have to replace it before
Taiyo Yuden make some very good media, but to be honest, they are something that I feel needs to be researched a bit more, in fact, its a bit of a shame, but making DVD's is an area that altogether needs to be researched a bit more to get the best from them. Its not like most areas of the computing world where you can just plug in and go and your ok.
But, the most basic thing I say to friends that say to me "I dont care about this MID/media/maker/factory thing just tell me the easy way to get the best disk I can..." I say stick with Verbatim, and I expect that most of the regs here will say the same thing when asked like that.
If you do care and want to do the best for yourself then you should start with the sticky threads at the top of this forum, and at the least read the Media FAQ and the Taiyo Yuden FAQ thread and get ready to do some searching and reading, its not an impossible task by any means, just not something that can easily be got across, and written down in less than this amount of words.

Just use Verbatim DVD's + or - and dont burn too fast.
When I say just use Verbatim to people, I also say that going too fast is often a cause of problems and stick to something like 8x and use Verbatim media and life will be much easer.
