Anyone familiar with LG return procedure?

I bought a LG 4040 in Dec 2003, a few days ago it gave “tracking servo failure” error and yesterday it finally gave up on DVD, the drive will not see any DVD media, CD is ok.

I called LG service this morning and they asked a few questions and then faxed me a form and ask me to fax it back with the receipt of LG 4040, I chose the option "ship replacement unit first (require credit card number to ensure return of defective unit) and faxed it back togather with the receipt, I haven’t heard from them since.

What’s next? anyone
