the dvd drive is teac dvd+rw dv-w58e in windows xp
the disks are memorex dvd+r 4xrw

most +R media has a RW logo, check the packaging does it say write once only?

I'm having the same problem with the Ritek 4X DVD+RW. Yes, it does say "DVD+RW".

have you tried using nero cd/dvd speed v3.5 to erase your discs? i find it does a better job that nero burning rom. or if that fails try using dvdinfopro.
