I have. I play from the virtual drive all the time. Do you have the DVD or CD version? If it's the CD version, just select the Safedisc 2/3 profile and let it read through the errors. If it's the DVD version, just create the image (no options to select).

I have as well, I haven't needed to use any blacklist fixes either. (I was using Daemon-Tools.) If your using a older version of Alcohol, try the newer one, just barley came out.
