PMA Update Failure?

What does "PMA Update Failure" mean? Does it reflect a disk problem or a drive problem?

When you begin writing to a disc, in Track-At-Once mode the Lead-In is reserved, but not written too as yet, as the data has yet to be written to the disc. And the Lead-in contains information as to where the data is located on the disc so that it can be retrieved later when you go to read the disc.

So does this apply only to Track-At-Once mode?

I suspect that in this case it's a media problem, but if it happens with different disks I may have to RMA the burner.
